the Lighting Supplement Questionnaire. Table A1 presents a brief
summary of the questions included in each of the questionnaires.
A complete list of all forms used in the 1993 Residential Energy
Consumption Survey follows. Household Energy Consumption and
Expenditures, 1993 contains a copy of each form.
